This week, the Department of Health Care Services (DHCS) launched
a new web portal, PATHways
to Success, which features success stories and testimonials
from organizations, including counties, participating in
the Providing Access and Transforming Health (PATH) initiative.
PATH
is a multi-year, $1.85 billion initiative intended to help
counties, community-based organizations, Medi-Cal providers, and
tribes build the capacity and infrastructure needed to strengthen
participation in the Medi-Cal delivery system as California
implements Enhanced Care Management (ECM), Community Supports,
and Justice-Involved services under CalAIM.
Counties and other organizations currently engaged in PATH are
encouraged to submit a PATH success story to be featured on
PATHways to Success. Stories should provide a clear
connection about how your agency’s participation in one of the
four PATH initiatives led to the successful delivery of
ECM/Community Supports. To submit a story,
