On November 12, 2024, the Department of Health Care Services
(DHCS) launched new resources to help providers succeed in the
California Advancing and Innovating Medi-Cal (CalAIM)
environment.
New On-Demand resources include:
Medi-Cal Managed Care 101 for Community Supports
Medi-Cal Managed Care 101 for Enhanced Care Management (ECM)
services
CalAIM Navigator video series
The resources are designed for organizations who are contracted
with or planning to contract with Medi-Cal managed care plans to
provide ECM and/or Community Supports services in California, and
will help them navigate the CalAIM space. These new resources are
available at the PATH
On-Demand Resource Library.
DHCS will also be hosting a public webinar on December 6,
2024 from 10:00 AM to 11:00 AM, “Hospital Engagement in
CalAIM: Supporting Connection to ECM Services Among Eligible
Medi-Cal Members.” The webinar will:
Explain and emphasize the value of hospitals engaging with
CalAIM providers in their region.
Recognize opportunities for improved collaboration on
discharge planning between hospital staff and ECM or Community
Supports providers.
Identify opportunities for warm handoffs and other
coordination strategies between hospital staff and ECM providers
to improve member connection to ECM and Community Supports
services.
The webinar is open to the public and advanced registration is
required.
