Upcoming Webinar: CalAIM Enhanced Care Management and Community Supports

The Department of Health Care Services (DHCS) will host a public webinar, “Tools to Better Engage Eligible Members in CalAIM,” on June 27, 2024, from 11:00 am to 12:00 pm as part of a series of Providing Access and Transforming Health (PATH) Collaborative Planning and Implementation (CPI) webinars. This webinar seeks to achieve the following objectives for participants:

  • Enhanced Care Management (ECM) and Community Supports providers learn how to adapt Job Aids to improve engagement of eligible members when describing ECM and Community Supports programs.
  • Local organizations recognize the benefit of a CalAIM coordinator position to connect systems of care and increase referrals and utilization.
  • ECM and Community Supports providers identify a strategy to establish warm handoffs and increase referrals with health care providers.
  • Healthcare providers identify an opportunity to better access and utilize client data for those experiencing homelessness through case finding.

The webinar is open to the public and geared towards state and local government employees, community-based organizations, ECM and Community Supports providers, and Medi-Cal managed care plan (MCP) staff. Register here.

The PATH CPI Initiative provides funding for county and regional collaborative planning efforts to support implementation of ECM and Community Supports. The PATH CPI webinar series is designed to highlight best practices for implementing ECM and Community Supports, improve collaboration among Medi-Cal managed care plans and government agencies, and deliver quality services to Medi-Cal members.
